29 CFR 1910.23(b)(13):29 CFR 1910.23(b)(13):The employer did not ensure that no employee carries any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder. On or about March 15, 2022, the employer failed to ensure employees climbing a ladder did not carry a load that could cause the employee to lose their balance. Employees are required to access the top of the buildings to perform cleaning and maintenance of the hoods and ducts. They are exposed to falls of varying heights based on the height of the structure, in this case approximately 10-15 feet.

29 CFR 1910.30(a)(1):29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. On or about March 15, 2022, the employer failed to ensure employees had been trained to understand the nature of fall hazards in the work area and how to recognize them, prior to performing work from ladders. Employees are required to access the top of the buildings to perform cleaning and maintenance of hoods and ducts. They are exposed to falls of varying heights based on the height of the structure, in this case approximately 10 - 15 feet.
