29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Employees engaged in fabrication activities at the Stellar Industries facility located at 357 Rd 3168 Hastings, Nebraska, were exposed to an electrical hazard in that an electrical box, which was intended for use while permanently fixed to a wall and which had pre-punched knockouts, was attached to the end of a flexible cord and was used to power the Edwards Iron Worker.
