29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a) On or about March 16, 2022 at the construction site located at 2828 N Humboldt Blvd, Milwaukee WI, in the southeast trench employees were performing work in an approximately six foot deep trench without cave-in protection, thereby exposing employees to a struck-by / crushing hazard. (b) On or about March 16, 2022 at the construction site located at 2828 N Humboldt Blvd, Milwaukee WI, in the southwest trench an employee was performing work in an approximately six foot deep trench without cave-in protection, thereby exposing the employee to a struck-by / crushing hazard. (c) On or about March 16, 2022 at the construction site located at 2828 N Humboldt Blvd, Milwaukee WI, in the northeast trench an employee was performing work in an approximately seven foot deep trench without cave-in protection, thereby exposing the employee to a struck-by / crushing hazard.
