29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: a) On or before March 22, 2022, and at times prior, an unknown liquid chemical spill was left on the floor in the tool room area. This condition exposed employees to slip hazards while working in the tool room area. b) On or before March 22, 2022, and at times prior, hydraulic fluid spills were left on the floor in the screw machine area. This condition exposed employees to slip hazards while working in the screw machine area. c) On or before March 22, 2022, and at times prior, employees worked in the rail section area with uneven concrete floors. The uneven floor was covered with loose boards. This condition exposed employees to trip hazards while working in the rail section area.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: a) On March 22, 2022, and at times prior, two compressed gas cylinders were used and stored by the employees in the tool room area. The compressed gas cylinder was stored unsecured and could tip or fall over. This condition exposed employees to serious hazards.

A lockout device was not utilized during the lockout/tagout process to prevent power from energizing the machines. This condition exposed employees to serious hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or before March 22, 2022, and at times prior, employees used four lathe machines and two surface grinders in the tool room area. The guard cover was missing on the point of operation of the lathe machines. This condition exposed employees to flying chips while using the lathe machines. b) On or before March 22, 2022, and at times prior, employees used three vertical drill presses in the tool room area. The guard cover was missing on the point of operation of the drill presses. This condition exposed employees to flying chips while using the drill presses. c) On or before March 22, 2022, and at times prior, employees used a CNC machine in the rail section area. The interlock system of the machine was bypassed. This condition exposed employees to struck by hazards while using the CNC machine.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: a) On or before March 22, 2022, and at times prior, several containers with unknown chemicals in the workplace were not labeled, tagged, or marked with their product identifier and words. This condition potentially exposed employees to serious hazards from products that contain unknown chemicals.
