1910.212(a)(3)(ii) The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a) Armature Department - Stuffer machine (Machine ID# 1574): On or about March 23, 2022, the employer did not ensure that a moving metal slide (point of operation) was safeguarded during normal production operation. The unaffixed, hinged plexiglass part did not prevent employees' access into the point of operation during removal of armature(s) in the danger zone. Employees had their fingers and hands exposed to caught-in hazards likely to result in serious injury. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form. On or about March 2, 2022, the employer failed to electronically submit their OSHA Form 300A Summary of Work-Related Injuries and Illnesses for calendar year 2021.
