29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about March 23, 2022, on the south side of the site, employees performed roofing tasks on a roof 6 feet (1.8 m) or above a lower level without adequate fall protection. The roof was approximately 6-12 pitch.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about March 23, 2022, on the south side of the site employees were performing roofing tasks while using an extension ladder whose side rails did not extend at least three (3) feet above the upper landing of the surface being accessed.
