29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel.  a)	USS Eisenhower: On or about 03/23/2022, an employee plugged an extension cord into the 115-volt receptacle outlet of the vessel to supply power to a portable vacuum cleaner and exhaust unit without ground fault circuit interrupter protection, as a result was exposed to a potential electrical shock hazard.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:  a)	USS Eisenhower: On or about 03/23/2022, an extension cord used to power portable electrical equipment was routed through a doorway causing the door to pinch the outer jacket of the cord.   No abatement certification or documentation required.

29 CFR 1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  a)	Quality Coatings of Virginia 3900 Holland Road Chesapeake, VA 23323: On or about 03/22/2022, the employer failed during calendar year 2022, to electronically submit information from their OSHA Form 300A or equivalent by March 02, 2022.
