29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  The written respiratory protection plan was not worksite specific.  The plan did not include specific procedures and elements for respirator use such as which respirator was required during which job tasks.  Employees were exposed to respiratory hazards.

29 CFR  1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter:  a) At least one firefighter that was required to wear a Scott full-face respirator was not fit tested when assigned to wear the respirator. The firefighter was exposed to respiratory hazards.  b) Firefighters were required to wear 3M full-face respirators and were not fit tested. The firefighters were exposed to respiratory hazards.  c) Firefighters were required to wear 3M N-95 respirators and were not fit tested. The firefighters were exposed to respiratory hazards.

29 CFR  1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer:  Shared Scott SCBA full-face respirator regulators were not cleaned and disinfected.  Employees used wipes to clean the full face respirators, but did not follow the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer.  Employees were exposed to respiratory hazards.
