29 CFR 1926.451(c)(2)(v):Fork-lifts were used to support scaffold platforms where the entire platform was not attached to the fork.  The employer failed to protect all employees exposed to fall hazards when conducting residential construction operations. This was most recently documented on or about March 24, 2022 at a residential construction jobsite located at 9915 S 181st St., Omaha Nebraska. The employer did not ensure that a supported scaffold platform that was being used with a powered industrial truck was secured to the forks.

29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.  The employer failed to protect employees from fall hazards when conducting framing operations on a residential structure while working from elevated mobile scaffolding. This was most recently documented on or about March 24, 2022 at a residential construction jobsite located at 9915 S 181st St., Omaha Nebraska. The exposed employees were not utilizing any form of fall protection when working from the elevated mobile scaffolding that was not fully guarded at a height of approximately 11.9 feet.

This was most recently documented on or about March 24, 2022 at a residential construction jobsite located at 9915 S 181st St., Omaha Nebraska. The exposed employees were working on the unfinished roof of the residential structure at heights over 6 feet and not utilizing any form of fall protection.

An employee was on the top step of a 6-foot stepladder while performing residential construction work.
