Facility:  64 Performance Drive Operation Company LLC DBA Care One of Weymouth  On or about  March 23, 2022, the employer did not record, including but not limited to,  the following work-related illnesses on the OSHA Form 300 or equivalent for the calendar year 2022:    a. A healthcare worker tested positive for COVID-19 on 1/29/22 after working with a positive co-worker and working  on a unit with multiply COVID-19 positive residents.  Worker had a minimum of five lost work days.  b. A  healthcare worker tested positive for COVID-19 on 1/31/2022  after working on a unit  with  multiple COVID-19 positive residents. Worker had a minimum of ten lost work days.  c. A dietary worker tested positive for COVID-19 on 1/31/22 after working with staff that tested positive for COVID-19 and  working on a unit with multiple COVID-19 positive residents. Worker had a minimum of five lost work days.   d.  A healthcare worker tested positive for COVID-19 on 2/2/22  after working on unit with multiple COVID-19 positive residents and working with staff who have tested positive for COVID-19. Worker had a minimum of five lost work days.   e.  A healthcare worker tested positive for COVID-19 on 2/2/2022 after working with a resident who tested positive for COVID-19. Worker had a minimum of five lost work days.  f. A healthcare worker tested positive for COVID-19 on 2/14/2022 after working on unit with multiple COVID-19 positive residents.
