Facility-Wide: On or about January 31, 2022, the employer's COVID-19 log did not contain employee contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace, and the date the employee first had one or more COVID-19 symptoms of the symptoms provided in their Line list.
