29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. The employer is failing to protect employees from fall hazards. This was most recently documented on or about March 25th, 2022 in Papillion, Nebraska. An extension ladder was used for access and egress to the upper level of the residential structure and it did not extend at least 3 feet above the surface being accessed. Also, the ladder was also not secured at its top to a rigid support.
