29 CFR 1910.134(f)(2):The employer did not ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. David Grant Medical Center: The employer did not provide an annual fit test to all employees required to wear respirators. The employer required employees to wear 3M Model Vflex 1804 N95 filtering facepieces and/or 3M NIOSH-approved 1804 respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 patients. The David Grant Medical Center was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1523750, citation number 01, item number 001 and was affirmed as a final order on July 7, 2021, with respect to a workplace located at 101 Bodin Circle Bldg. 777. Travis AFB, CA.
