29 CFR 1926.602(c)(1)(vii): Unauthorized personnel were permitted to ride on powered industrial trucks or a safe place to ride was not provided where riding of trucks was authorized. On or about March 23, 2022, one employee had not been provided with a safe place to ride on a power industrial truck when the employee was installing sheet metal to a storage unit under construction. The employee was standing on a pallet that was placed on the forks of a JCB forklift while they were being elevated approximately 12 feet from the ground exposing the employee to a potential fall.

29 CFR 1926.453(b)(2)(v): A body belt had not been worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about March 23, 2022, one employee was in an aerial lift measuring a section of the storage unit to install sheet metal without the use of a harness and lanyard, exposing the employee to a fall of approximately 12 feet to the ground below.

29 CFR 1926.760(a)(1): Each employee engaged in steel erection activity who was on a walking/ working surface with an unprotected side or edge more than 15 feet above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. On or about March 23, 2022, an employee secured sheet metal panels and materials to the roof of a storage building under construction due to a concern for high winds without using a fall protection system, exposing the employee to a fall hazard of approximately 16 feet.
