29 CFR 1910.178(l)(4)(ii)(B): Refresher training and evaluation was not provided to the operator when the operator was involved in an accident or near-miss incident. (a) 615 Graham Street Harrisburg PA Warehouse - The employer did not provide refresher training and evaluation after an employee ran into racking that housed flooring material with a forklift causing the product to fall, on or about March 25, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
