(b) On or about March 25, 2022, the energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking, and securing hazardous energy in the form of gravity (weight of machines components) on the Fosber roll stand machine.

29 CFR 1910.147(d)(5): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy was not relieved, disconnected, restrained, or otherwise rendered safe. (a) On or about March 20, 2022, three maintenance employees changing out a hydraulic cylinder on the Fosber roll stand machine which was locked out were exposed to an amputation hazard in that the hazardous energy of gravity due to the weight of the unsecured cylinder was not restrained or otherwise rendered safe.
