29 CFR 1926.1053(b)(1):Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Roof of building, at 47 Holly Spring Circle Port Wentworth, GA: on or about March 25, 2022, the employer exposed employees to fall hazards, in that the ladder did not extend 3 feet above the upper level and there was no grasping device to assist employees in mounting and dismounting the ladder.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Roof of building, at 47 Holly Spring Circle Port Wentworth, GA: on or about March 25, 2022, the employer exposed employees to a fall hazard, in that fall protection was not used by employees while performing roofing work. Roofcrafters Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number1299733, citation number 01, item number 01 and was affirmed as a final order on 04/11/2018, with respect to a workplace located at 5406 Buffalo Ave, Jacksonville, FL 34110.
