Employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) On or about March 28, 2022, employees were exposed to a fall of approximately 14 feet while performing framing activities on a residential home located at 17320 Corbin, Chagrin Falls, Ohio 44023. Employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area.
