29 CFR  1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.   a).  On or about March 18, 2022, the employer did not ensure procedures were developed, documented, and/or utilized for the control of potentially hazardous electrical and pneumatic energy of the National Machine Company, Bolt Maker Machine No. 124, while employees were engaged in service and maintenance activities, such as but not limited to, cleaning the wire feed rolls.  b).  On or about March 28, 2022, the employer did not ensure procedures were developed, documented, and/or utilized for the control of potentially hazardous electrical and pneumatic energy of the National Machine Company, Bolt Maker Machine No. 124, while employees were engaged in service and maintenance activities, such as but not limited to, set up operations.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a).  On or about March 18, 2022, the employer did not ensure an employee performing service and maintenance activities on the National Machine Company, Bolt Maker Machine No. 124 door applied energy controls by; turning off or shutting down the machine[per 1910.147(d)(2)], isolating the electrical and hydraulic energy sources [(d)(3)], affixing lockout device(s) to the energy isolation devices [(d)(4)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to cleaning the wire feed rolls.    b).  On or about March 28, 2022, the employer did not ensure employees performing service and maintenance activities on the National Machine Company, Bolt Maker Machine No. 124 door applied energy controls by; turning off or shutting down the machine[per 1910.147(d)(2)], isolating the electrical and hydraulic energy sources [(d)(3)], affixing lockout device(s) to the energy isolation devices [(d)(4)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to setting up the machines.

29 CFR  1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  On or about March 28, 2022, the employer did not ensure bolt machine operators, authorized to perform service and maintenance tasks on the National Machine Company, Bolt Maker Machine No. 124 (BM124), were trained in the recognition of hazardous energy sources ,type and magnitude of the energy available in the workplace, and methods and means necessary for energy isolation and control while performing setup operations and cleaning operations on the machine.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   a).  On or about March 28, 2022, the employer did not ensure one or more methods of machine guarding were provided to protect the operator of a National Machine Company, Bolt Maker Machine No. 124 (BM124), from the machine area hazards created by the point of operation of the roll former dies, rotating parts and ingoing nip points of the wire feed rolls, and moving and rotating parts of the kick arms.      b).  On or about March 28, 2022, the employer did not ensure one or more methods of machine guarding were provided to protect the operator of a National Machine Company, Bolt Maker Machine No. 120 (BM120), from the machine area hazards created by the point of operation of the roll former dies, moving and rotating parts of the bolt transfer tube and arm, rotating parts and ingoing nip points of the wire feed rolls, and moving and rotating parts of the kick arms.
