29 CFR  1910.178(q)(10): Industrial trucks were not kept in a clean condition, free of lint, excess oil, and grease. Noncombustible agents should be used for cleaning trucks. Low flash point (below 100 deg. F.) solvents shall not be used. High flash point (at or above 100 deg. F.) solvents may be used.  Precautions regarding toxicity, ventilation, and fire hazard shall be consonant with the agent or solvent used.  a) On or about March 28, 2022, the employer did not ensure the Linde model H70D-396 forklift windshield was in a clean condition prior to use. Hydraulic fluid was used to clean the windshield.

29 CFR  1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a. Plant wide - On or about April 13, 2022,  employees in the facility were provided 3M particulate respirator P95  for voluntary use. The employer did not provide the information contained in 29 CFR 1910.134, Appendix D ("Information for Employees Using Respirators When Not Required Under the Standard").
