29 CFR  1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:  On or about March 24, 2022, the employer did not ensure that the floor was maintained free of hazards and damaged.   a) Department B - There was a floor hole in the back aisle in Department B, near product bay B0339-A-01.   b) Department C -  There was a floor hole at the entrance of Department C, near product section C0001.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.41(a)(2): The establishment, which had 20 or more employees but fewer than 250 at any time during the previous calendar year and was classified as a high risk industry listed under Appendix A to Subpart E of this part, did not electronically submit information from the OSHA 300A, Summary of Work-Related Injuries and Illnesses, to OSHA or OSHA's designee once a year, no later than the date listed in paragraph (c) of this section:  a) The Bazaar, Inc., located at 1900 N. 5th Avenue, in River Grove, IL - On or about March 24, 2022, the employer had not electronically submitted information from calendar year 2021's OSHA Form 300A or equivalent by March 2, 2022. The establishment employed 90 employees onsite and was classified under NAICS code 424990 during calendar year 2021.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1910.176(b):Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  a) Department A - On or about March 24, 2022, the employer did not ensure material stored on pallets in racking systems were stable and secure, including boxes stored in the middle rack of product bay A0336 and the lower rack in product bay A0371.  b) Department D - On or about March 24, 2022, the employer did not provide a secure area for storage of products in Department D, Section D812. The boxes on the top pallet were not secured and the product was leaning toward the next product aisle where it could collapse onto employees.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET
