29 CFR 1926.451(c)(1)(ii):Guys, ties, and/or braces were not installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide: On or about 3/28/2022, on the scaffolds located at the jobsite where employees were applying Sto Gold Coat water proofing to the exterior of the building. The scaffold, measuring 3 feet wide, was tied to the building at 20 feet and 38 feet, instead of 12 feet and 32 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about 3/28/2022, on the scaffolds located at the jobsite where employees were applying Sto Gold Coat water proofing to the exterior of the building in the following instances: a-j) East end and south side of levels 2-6 of the scaffolds erected on the north end of the east side; k-o) North end of levels 2-6 of the scaffold erected on the East side; p-s) Center section, east side, levels 3-6; t-w) North end of East side center section, levels 3-6; x-bb) South end of East side center section, levels 2-6; cc-gg) South end of East side, levels 2-6; hh-ll) Scaffold erected on south end, east end, levels 2-6; mm-xx) Top level, seven platforms; yy-ccc) South end, west end of levels 2-6.
