29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) On or about 3/18/2022, and at times prior to, 2100 Market Street, Decatur, AL in the Aluminum Extrusion Plant at the Press 2 Die Pusher Table, employees aligning the ram by wedging items between the ram and the die in the Die Pusher Table were exposed to caught-in and amputation hazards when accessing the point of operation to manually secure and align the die and ram with items such as, but not limited to, hammers, metal butts, or wood blocks.
