29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator or annually refitted: a) At facility, 130 W Kingsbridge Rd Bronx, NY 10468: James J. Peters VAMC did not fit test a register nurse who is required to wear a3M 1860 N95 respirators when providing direct care to patients infected with COVID-19. Condition noted on or about 03/30/2022. Repeat Citation: James J Peters VA Medical Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(2) which was contained in OSHA inspection number 1478341 citation number 1, item number 1(b) and was affirmed as a final order on 12/21/2020, with respect to a workplace located at 130 W. Kingsbridge Rd Bronx, NY 10468. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
