29 CFR 1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: On or about March 30, 2022, at this location, employees conducting roofing activities utilized an extension ladder to access/egress the roof. The ladder did not extend at least three feet over the roof landing, exposing employees to fall hazards.
