29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part: a) Jobsite located at 1201 Girard Avenue, Philadelphia, PA.- The employer did not maintain an accident prevention program to help protect their employees from hazardous working conditions such as, but limited to, hazards associated with excavation operations. Employees were exposed to soil collapse hazards while installing a new sewer service in an approximate 6-foot-deep trench without an adequate protective system. Condition was observed on or about March 31, 2022.

Condition was noted to have occurred on or about March 31, 2022
