Location - 1894 Revere Beach Parkway, Everett, MA.  On or about March 31, 2022, an employee was not adequately instructed in the recognition and avoidance of unsafe conditions while spotting back a flatbed semi trailer. The employee got caught in-between semi flatbed  and an excavator causing injury and hospitalization.
