29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. Location - 1894 Revere Beach Parkway, Everett, MA. On or about March 31, 2022, an employee was not adequately instructed in the recognition and avoidance of unsafe conditions while spotting back a flatbed semi trailer. The employee got caught in-between semi flatbed and an excavator causing injury and hospitalization.
