USPS Centralia 314 S. poplar St., Centralia, IL 62801 On or about 05-03-2022, at USPS Centralia, the employer failed to provide certified annual summaries of work-related injuries and illnesses for years 2019, 2020 and 2021. There was no signature on the Summary Forms, and no indication of an individual considered to be a company executive.

USPS Centralia 314 S. poplar St., Centralia, IL 62801 On or about 05-03-2022, OSHA Compliance Officer requested OSHA 300 and 300A logs for the employer and they were not provided within the four business hours required by the standard. CSHO received incomplete and unofficial 300 and 300A logs from the employer on or about 05-10-22.
