Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a. 1635 N Williamson Blvd., Daytona Beach: On or about March 31, 2022, on a 3:12 pitch roof, employees performing roofing operations, were not protected from a 14 foot fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.
