29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: a. On or about March 21, 2022, employees using ladders to access the roof are not protected from fall hazards, in that the employer is not enforcing the extension of the ladder 3 feet above the landing. The employer fails to assure effective protections against falls as required by the standard, as demonstrated on March 31, 2022, when 4 employees using a ladder to access a residential roof at 536 Revilo Boulevard, Florida, are exposed to an approximate 17 foot fall hazard to the ground in that the ladder does not extend 3 feet above the landing. G & W Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1533308, citation number 1, item number 2, and was affirmed as a final order on August 30. 2021, with respect to a workplace located at 115 Executive Boulevard, Daytona Beach, FL 32114.
