29 CFR 1910.243(c)(3): Abrasive wheels used on the vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel. On or about July 27, 2021, employees were using angle grinders without guards in the fabrication shop. A) On the south side fabrication shop, an employee used a Dewalt 4 1/2-inch angle grinder without an adequate guard while performing fabrication tasks. B) On the southeast side, an employee was using a Dewalt 4 1/2-inch angle grinder without an adequate guard while performing fabrication tasks.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to an outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On or about March 30, 2022, extension cords were used on the south side with damage to the outer insulation exposing the inner wiring.
