29 CFR 1910.132(d)(2):At this establishment, the employer had not verified that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and which identifies the document as a certification of hazard assessment. JOB SITE: 114 N. 2nd Street, Clarksville, MO 63336, on or about 4/4/2022, the employer had not verified that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and which identifies the document as a certification of hazard assessment. Employees were exposed to candle making hazards, handling and pouring hot candle wax into multiple jars in close proximity to each other without any form eye or face protection. Employees were exposed to potential burn hazards.

The workers were exposed struck-by hazard.

29 CFR 1910.305(b)(2)(i):Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: JOB SITE: 114 N. 2nd Street, Clarksville, MO 63336, on or about 4/4/2022, at the time of the inspection, junction boxes were not provided with cover and each outlet box in completed installations did not have a cover. The employees were exposed an electrical hazard. The following outlet boxes were missing covers: a) at the front counter, inside the lower section, an electrical junction box was missing a cover exposing the employees to approximately 115VAC. b) In the electric pallet jack charging system area, the outlet on the west wall, was missing a cover exposing the employees to approximately 115VAC.
