29 CFR  1910.29(f)(1)(i):  Handrails are not less than 30 inches (76 cm) and not more than 38 inches (97 cm), as measured from the leading edge of the stair tread to the top surface of the handrail.   On or about April 1, 2022, employees were exposed to fall hazard while ascending/descending a stairway that did not have at least one handrail.

29 CFR  1926.1060(a):  The employer did not provide a training program for each employee using ladders and stairways, as necessary. The program did not enable each employee to recognize hazards related to ladders and stairways, and did not train each employee in the procedures to be followed to minimize these hazards:  a)  On or about March 29, 2022, an employee was exposed to fall hazards while working from an extension ladder and had not been trained.
