29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an equivalent 8-hour time-weighted average of 85 decibels : Production Area: Employer did not obtain a new annual audiogram for each employee who were engaging in aluminum metal fabrication operations (extruding, cutting and stacking of products) and were exposed to noise in excess of the OSHA action level of 85 decibels (dBA). The following employees who were sampled, were exposed to noise at or above 85 dBA equivalent 8-hour time weighted average: a) A Press Operator was sampled for 565 minutes and was exposed to noise at 89.1 dBA. b) A Stacker was sampled for 572 minutes and was exposed to noise at 89 dBA.

29 CFR 1910.95(i)(3): Employees were not given the opportunity to select their hearing protectors from a variety of suitable hearing protectors provided by the employer: Production Area: Employer did not provide a variety of suitable hearing protectors and give the opportunity to select hearing protectors to employees who were engaging in aluminum metal fabrication operations (extruding, cutting and stacking of products) and were exposed to noise in excess of the OSHA action level of 85 decibels (dBA). The following employees who were sampled, were exposed to noise at or above 85 dBA equivalent 8-hour time weighted average and/or above the OSHA permissible exposure limit of 90 dBA: a) Press Operator was sampled for 565 minutes and was exposed to noise at 89.1 dBA. b) A Stacker was sampled for 572 minutes and was exposed to noise at 89 dBA. c) Rough Saw Cutter was sampled for 576 minutes and was exposed to 93.4 dBA. d) Finish Saw Cutter was sampled for 583 minutes and was exposed to 88.4 dBA.
