29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations: Location: 86 East Hanover Ave Cedar Knolls, NJ 07927 - Commercial workshop warehouse a) Employees were exposed to crushing hazards; improper demolition methods or procedures were conducted as the employer did not provide an engineering survey for the worksite to address hazards which ensured that an unplanned collapse of structure did not occur. The incident occurred on or about 4/2/22.

29 CFR 1926.854(b): Wall sections more than one story in height were permitted to stand alone without lateral bracing when the wall section was not originally designed to stand alone without such lateral support: Location: 86 East Hanover Ave Cedar Knolls, NJ 07927 - Commercial workshop warehouse a) Employees were exposed to crushing hazards while conducting concrete wall demolition to install an overhead garage door within a warehouse without any lateral bracing provided to support the wall to prevent it from collapsing. The incident occurred on or about 4/2/22.
