29 CFR  1910.147(a)(3)(i):The employer did not utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices, and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees:  Vertical Milling Machine Area: On or about April 4, 2022, lockout/tagout was not used on the Fagima Vertical Milling Machine # 1, type JAZZR-5AX, serial 834/18 nor on the Meccano Robot, type A05B-1335-B201, serial R18303153, while maintenance was performed, exposing employees to the hazards associated with energized machinery and equipment.

29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  203 Hartford Turnpike, Tolland, CT: On or about April 4, 2022, the lockout/tagout program, including the written program and machine specific procedures, hadn't been reviewed since June 2, 2014. The Meccano Robot, type A05B-1335-B201, serial R18303153 and the Fagima Vertical Milling Machine # 1, type JAZZR-5AX, serial 834/18, were placed in service between 2018 and 2019, and were not incorporated into the existing energy control procedures.
