29 CFR 1926.404(f)(6):The path to ground from circuits, equipment, or enclosures was not permanent and continuous. At a worksite located at 400 sw Nelson drive, Blue Springs, MO 64014, an employee was exposed to electrical hazards that included electrocution and electric shock. The employer exposed the employee to these hazards by allowing the use of an electrical extension cord set that had a missing ground pin that was powering a "skilsaw" circular saw being used on site.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. At a worksite located at 400 sw Nelson drive, Blue Springs, MO 64014, an employee was exposed to electrical hazards that included electrocution and electric shock. The employer exposed the employee to these hazards by allowing the employee to use an extension cord set that had damaged insulation, as well as damage associated with improper strain relief. The extension cord set was being utilized to power a skilsaw circular saw that was being used on the work site.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). At a worksite located at 400 Sw Nelson drive, Blue Springs, MO 64014, employees were exposed to fall hazards greater than six feet. The employer exposed employees to fall hazards by allowing them to work on the roof of a structure without appropriate fall protection or mitigation methods in place. CJJ Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1318754, citation number 1, item number 2 and was affirmed as a final order on 07/25/2018, with respect to a workplace located at 4337 Lakeview Ter., Basehor, KS 66012.
