On or about April 4, 2022, April Enterprises DBA Walnut Creek Nursing Center did not provide fit test records that included the type of fit test performed, the specific make, model, style, and size of the respirator tested, and the pass/fail results for the qualitative fit tests performed for employees required to wear N95 filtering facepiece respirators when caring for residents with suspected or confirmed COVID-19.
