29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:  a) Roof - Roofers were observed dumping materials over the edge of a flat roof 18 feet above ground and were not protected from fall hazards, on or about April 4, 2022.  Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.502(f)(1): Warning lines were not erected around all sides of the roof work area:  a) Roof - Employees were performing roofing activities on a flat roof with a warning line system that was not erected around all sides of the work area located 18 feet above ground. At the north end of the warning line system a 43 inch wide opening was observed between the stanchion and parapet wall, on or about April 4, 2022.  b) Roof - Employees were performing roofing activities on a flat roof with a warning line system that was not erected around all sides of the work area located 18 feet above ground. At the south end of the warning line system a 58 inch wide opening was observed between the stanchion and parapet wall, on or about April 4, 2022.  No abatement certification or documentation required.

29 CFR 1926.502(f)(1)(ii): When mechanical equipment was being used, the warning line was not erected at least 6 feet (1.8 m) from the roof edge which is parallel to the direction of mechanical equipment operation, and at least 10 feet (3.1 m) from the roof edge which is perpendicular to the direction of mechanical equipment operation:  a) Roof - Roofers tore off sections of the roof using an Enforcer Twin Saw and the warning line was not erected at least 10 feet from the roof edge which was perpendicular to the direction of mechanical equipment operation, on or about April 4, 2022.   No abatement certification or documentation required.

29 CFR  1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface.   a) Roof - At the north end of the warning line system the line measured  32 inches high from the walking/working surface of the roof, on or about April 4, 2022.   b) Roof - At the south end of the warning line system the line measure 18 1/2 inches high from the walking/working surface of the roof, on or about April 4, 2022.   No abatement certification or documentation required.
