29 CFR 1910.132(f)(3)(iii): Inadequacies in an affected employee's knowledge or use of assigned PPE indicate that the employee has not retained the requisite understanding or skill:  a.) Glenn O. Hawbaker, Inc,; Asphalt Yard; dry drum/mixing drum - On or about 4/1/22 and at times prior thereto, employees were lifting a chute into place in the dry drum area of the plant.  Employees were using a come along attached/locked to a lifting lug on the chute cover. An accident occurred on 4/1/22 when the lifting lug, which was attached to the come-along, broke off the chute and caused the chute to fall. The chute dropped down approximately 2-3 feet, striking an employee who sustained a fracture injury.  The lifting lug was welded to the chute cover improperly in a tack weld fashion.   b.) Glenn O. Hawbaker Inc.; Asphalt yard; dry drum/mixing drum - On or about 4/1/22 and times prior there to, employees were lifting a chute into place in the dry drum area of the plant.  Employees were using a come-along attached/locked to a lifting lug on the chute's cover.  The chain of a Harrington hand chain hoist (Code: CF010 / M/N: CF4-0222 / S/N: B003766) was wrapped around the chute and being utilized as a sling. The load was improperly rigged in that the hoist chain was wrapped around the load.   ABATEMENT CERTIFICATION & DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
