29 CFR 1910.147(f)(1)(v): Deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue the servicing and/or maintenance. a) Pale Assembly Department - On or about March 28, 2022, an employee was conducting servicing and maintenance on an Ear Welder machine. While troubleshooting the ears inside the machine to make sure they were lined up with the holster, the machine cycled while the employee was inside the machine. The employer did not ensure that employees troubleshooting the equipment deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue servicing the equipment.
