29 CFR 1910.106(e)(6)(i):  Adequate precautions were not taken to prevent the ignition of flammable vapors:  On March 29, 2022, and times prior to, employee(s) of Craftsmen Industries, Inc. located at 3101 Elm point Industrial Drive, Saint Charles, MO did not ensure that precautions were taken to control flammable vapors during de-fueling operations.  A Milwaukee M18 transfer pump, Model # 2771-20, Serial # H59BB211802507A, was used to transfer diesel fuel.  The pump was not approved for use with flammable liquids.  Employees were exposed to fire and explosion hazards.

TCM, Model # FCG25F9, Serial # A12W01106, was equipped with a set of fork extensions measuring 72 inches long.  Forks without extensions measure 41 inches long.

CAT, Model # GC45IL-LP-SWP, Serial # AT87A30432, in need of repair had not been removed from service, the alignment pin for liquified petroleum gas tank was excessively bent. b.) TCM, Model # FCG25F9, Serial # A12W01106, in need of repair had not been removed from service, the horn was not functioning, and the gas and clutch pedal were missing non-slip covers.

29 CFR 1910.252(b)(2)(iii):  Workers and other persons adjacent to the welding area were not required to wear appropriate googles, to protect them from rays when noncombustible or flameproof screens or shields were not used:  Production Area - On April 4, 2022, welding assistants and other employees in the vicinity of welding operations were exposed to welding rays due to lack of appropriate googles, screens or shields.  Employees in these areas were exposed to skin and eye damage due to flash burns.

Secondary containers containing chemicals were not used in their entirety during the current work shift, requiring the container to have a label.
