Jobsite - On or about April 5, 2022, employees onsite were not trained in the recognition and avoidance of unsafe conditions. Employees were working on the roof not utilizing fall protection, using a table saw with no blade guard, and exposed to electrical injuries through use of a damaged electrical cord and improper use of relocatable power taps.

29 CFR  1926.304(i)(1): Each circular had-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.  Jobsite, Rigid portable table saw - On or about April 5, 2022, the table saw being used to rip boards did not have a guard installed that enclosed the portion of the blade above the table and above the material being cut. The guard had been removed from the saw completely.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   Jobsite, office area roof- On or about April 5, 2022, employees engaged in roofing activities were not protected from falls of approximately 18 feet from the front of the roof and 7 feet from the back of the roof,  where fall protection was not utilized.

Jobsite - On or about April 5, 2022, employees onsite were not trained on the hazards of falling or the procedures to be followed to minimize the hazards. Employees were exposed to a fall of approximately 18 feet from the office roof where fall protection was not installed or utilized.
