Jobsite - On or about April 5, 2022, employees onsite were not trained in the recognition and avoidance of unsafe conditions. Employees were working on the roof not utilizing fall protection, using a table saw with no blade guard, and exposed to electrical injuries through use of a damaged electrical cord and improper use of relocatable power taps.

Jobsite, Rigid portable table saw - On or about April 5, 2022, the table saw being used to rip boards did not have a guard installed that enclosed the portion of the blade above the table and above the material being cut. The guard had been removed from the saw completely.
