Coil Painting Line - On or about April 1, 2022, the employer failed to ensure the 5-step stairway used by employees to access the top of tank #2 to perform tank cleaning was equipped with a stair rail system with a handrail on the open side, thereby, exposing employees to a four (4) feet, eight and one-half (8 1/2) inch fall hazard.

Coil Painting Line - On or about April 1, 2022, employees required to access the top of tank #2 to perform tank cleaning were exposed to falls of approximately six (6) feet, five (5) inches, created by the presence of unprotected sides.

Coil Painting Line - On or about April 1, 2022, the employer failed to ensure an energy control program consisting of an energy control procedure, employee training and periodic inspections was established for employees performing maintenance activities, including entering ovens #1 and #2 and their related igniter spaces, to perform cleaning.   Affected employees were, thereby, exposed to potential thermal and atmospheric (oxygen displacement) hazards when oven controls and gas valves were not isolated (locked out) prior to entering the spaces.
