29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Worksite located at Lot #138 Cordavelle Ct., Columbia, Missouri- Employees were exposed to overhead hazards from materials and equipment being thrown up and down to employees working above the employees. The employees on the ground were not protected from impact by the use of hard hats.

29 CFR 1926.403(b)(1):Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: Worksite located at Lot #138 Cordevalle Ct., Columbia, Missouri- The employer failed to inspect cords prior to use. Employees were exposed to a shock hazard in that the extension cord the employees were using had its' strain relief pulled away and several breaches to the outer insulation.
