29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Amentum Services Inc.: On or before 4/4/22, the employer did not protect employees from injury in that the MPRS saddle was manually loaded with a dummy round during the diagnostic phase. The system was in "engineering mode," which allowed the system to automatically run subroutines while being manually advanced. When the dummy round was placed in the MPRS saddle, a clamp actuated to secure the dummy round and crushed the operator's finger, as the operator had not let go of the dummy round. Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."
