29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At a worksite located at 27612 Burrows Street (Lot 18), Blue Springs, MO 64015: Employees were exposed to electrical shock hazards. This was demonstrated on April 5, 2022, when employees worked with a reciprocating saw that had strain relief that was present. Employees were working to install new water and sewer lines to a new residential home and were not provided with equipment that was free of hazards before it was used to cut sections of the new sewer line.

29 CFR  1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  At a worksite located at 27612 Burrows Street (Lot 18), Blue Springs, MO 64015: Employees were exposed to struck-by and crushing hazards. This was demonstrated on April 5, 2022, when employees worked in an unprotected excavation where excavated materials were placed at least 2 feet or less from the edge of the excavation. Employees were working to install new water and sewer lines to a new residential home and were not provided with protection from falling materials by placing the material at another location or further than 2 feet from the excavation edge.

29 CFR 1926.652(a)(1)(ii): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph 29 CFR 1926.652(b) or 29 CFR 1926.652(c) of this section except when: excavations are less than 5 feet (1.52 m) in depth and examination of the ground by a competent person provides no indication of a potential cave-in.    At a worksite located at 27612 Burrows Street (Lot 18), Blue Springs, MO 64015: Employees were exposed to struck-by and crushing hazards. This was demonstrated on April 5, 2022, when employees worked in an unprotected excavation 5 feet 11 inches in depth. Employees were working to install new water and sewer lines to a new residential home and were not provided with any benching, sloping, or other engineered protective systems.
