Worksite located at  4877 Osage Parkway Ave., Osage Beach, Missouri- Employees were exposed to fall hazards while working on the exterior platforms which were not protected by adequate guardrails. The host employer failed to install midrails on and therefore exposed all employees on the jobsite to fall hazards.  (b). Worksite located at 4877 Osage Parkway Ave., Osage Beach, Missouri- The employer staged his mixing pumper next to a wall where there was a fall hazard in which employees were not protected from falling by the use of any means of fall protection.
