(a) Columbine Commons Health & Rehab Center at 1475 Main St., Windsor, CO 80550; On April 8, 2022 the employer failed to provide copies of the injury and illness records to an authorized representative within four hours of the request. Injury and illness records were provided on June 30, 2022.

29 CFR 1904.7(b)(3): An injury or illness involving one or more days away from work, was not recorded on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known: (a)Columbine Commons Health and Rehab LLC: On or about June 13, 2022 the employer did not record two incidences of COVID-19 illness contracted while at work, which resulted in days away from work. The incidences were not recorded with a check mark in the space for cases involving days away from work and an entry of the number of calendar days away from work each employee was away from work due to the illness.

29 CFR 1904.29(b)(3):Each recordable injury or illness was not entered on the OSHA 300 Log and on an incident report (OSHA Form 301or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness had occurred: a) Columbine Commons Health and Rehab LLC: On or about June 21, 2022 the employer failed to enter work related illnesses on the OSHA 300 log within 7 days of the recordable incidences. Two employees contracted COVID-19 as a result of workplace exposure, which were not entered on the OSHA 300 log for the site.
