29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. At a residential worksite located at 8515 McKinney Ave, Kansas City, Missouri--Employees were exposed to serious eye injury while using a Hibachi brand pneumatic nail gun without eye protection.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a residential worksite located at 8515 McKinney Ave, Kansas City, Missouri. Employees were exposed to a fall hazard while working at heights of approximately 11 feet above the surface below without proper fall protection to include guardrail systems, safety net systems, or personal fall arrest system.

29 CFR  1926.501(b)(14):Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  At a residential worksite located at 8515 McKinney Ave, Kansas City, Missouri. Employees was exposed to a fall hazard while working next to unprotected wall opening at heights of approximately 12 feet above the surface below without proper fall protection to include guardrail systems, safety net systems, or personal fall arrest system.
